Cardiovascular disease — heart attack, stroke and heart failure — is responsible for around 160,000 deaths in the UK each year. Yet up to 80% of premature cardiovascular deaths are preventable through addressing modifiable risk factors. Home health monitoring is an increasingly important part of that prevention.

Understanding Cardiovascular Risk

Your cardiovascular risk is not determined by any single factor but by the interaction of multiple risk factors. The NHS QRISK3 calculator considers: age, sex, blood pressure, cholesterol, smoking, diabetes, ethnicity, family history of heart disease, social deprivation, BMI, and several medical conditions. People with a 10-year cardiovascular risk above 10% are typically offered statin therapy and lifestyle advice by their GP. Knowing your risk factors — including your blood pressure — is the first step to managing them.

Home Blood Pressure Monitoring

Home blood pressure monitoring (HBPM) has several advantages over clinic measurements: it eliminates white-coat hypertension (elevated readings due to anxiety in clinical settings), provides a more representative picture of habitual blood pressure, and allows tracking of lifestyle interventions and medication effects. NICE recommends HBPM to confirm the diagnosis of hypertension before starting treatment, and for ongoing monitoring. Wrist monitors are significantly less accurate.

Why the Average Matters More Than Any Single Reading

Blood pressure fluctuates throughout the day — affected by activity, stress, caffeine, full bladder, and time of day. A single elevated reading means little in isolation. Clinical guidelines recommend taking at least 3 readings over multiple days and using the average. For HBPM-based diagnosis, NICE recommends at least 2 readings twice daily for 7 days and averaging all readings except the first day’s.

Key Lifestyle Changes That Lower Blood Pressure

Dietary sodium reduction is the most evidence-based lifestyle intervention — aiming for below 6g of salt per day (the average UK intake is ~8g). The DASH diet (high fruit, vegetables, low-fat dairy, whole grains, reduced sodium and alcohol) reduces systolic blood pressure by 8–14mmHg in clinical trials. Physical activity (150 minutes of moderate activity per week) reduces blood pressure by 4–8mmHg. Weight loss (1mmHg reduction per 1kg lost). Limiting alcohol (below 14 units per week). Stopping smoking. Stress management — techniques that reduce chronic stress hormone output (cortisol, noradrenaline) have genuine BP-lowering effects.

Other Monitors Worth Having at Home

Pulse oximeters measure blood oxygen saturation (SpO2) and heart rate. Normal SpO2 is 95–100%; below 95% warrants medical attention. Became widely used during COVID-19 and remain useful for those with respiratory conditions. Peak flow meters track lung function in asthma — vital for early detection of deteriorating asthma control. Cholesterol testing kits — available from pharmacies; provide a general guide but are less accurate than laboratory tests.
